Copy of current DEA certificate (and controlled substance registrations, if applicable)

Curriculum vitae, including up-to-date work history

Copy of medical school diploma

Copy of internship, residency, and fellowship certificates

Copy of ECFMG certificate (if foreign medical graduate)

Copy of ACLS, BCLS, PALS, ATLS (if applicable)

Copy of current malpractice coverage

Documentation of any malpractice, licensure or other actions (if any)
